Diesel loosing power on incline and blowing clouds of black smoke?

I own an 04 Peugeot Partner 1.9 diesel and it now looses power on hills and starts blowing great clouds of black smoke.Changing down doesn't seem to improve things.Its getting embarring,recently I had to change down to first and was only doing about 15 mph and buses were overtaking me.HELP.
