Childrens book about a mother and Son?

There is an adorable children's book about a mother and her son. She sings a lullaby to him each night as he is growing up. And then he moves out of the house and his mom becomes sick so he goes and sings her the same lullaby. I can't for the life of me remember the books title!!! So if any one knows i would love to know too!!!!
